In order to keep the cost of medical care to a minimum, we request payment for all office services at the time they are rendered, unless prior arrangements have been made. If we are a participating provider with your insurance plan, we will bill them for the services we provide. All applicable co-pays and deductibles are the patient’s responsibility and are due at the time of service.

We will assist in any way we can with payment arrangements if you so desire – ask for our billing manager if you need assistance.
